摘要
我国人口基数大，目前正在向着老年人社会快速踏进，老年人的赡养问题和精神问题受到了社会的广泛关注和高度重视。城市居家老人主要依靠自己儿女赡养，但是在我国生育政策的影响下，仅仅依靠自己儿女赡养很难满足他们的身心需求，加之，受传统养老观念的影响，家庭养老出现了系列问题，老人精神状况令人担忧，尤其是心灵孤独很难得到排泄，影响着他们日常生活和社交活动。随着我国城市建设的快速推进和社区的快速发展，以社区为依托的新的养老服务方式也在酝酿形成当中，如何发挥社区的养老服务功能，已成为理论与实践研究重点探讨的问题，每个老年人都有不同的养老服务需求，因此，研讨城市社区居家养老需要从他们的心理需要出发，构建多层次、多种服务内容的养老方式。
本文通过广泛查询、收集、分析文献资料，及问卷调查法和访谈的方式对社区养老现状、问题及需求进行了调查和分析研究，发现了很多老人不愿意去养老机构，而是选择居家养老的方式度过晚年，面对着这种情形，城市社区居家社会工作介入就显得很有必要，因此，成为本次社区居家养老重点研究的问题，运用行政体系下社会工作介入与专业分工，综合人才介入、制度介入、理念介入和服务介入方式，构建社区居家养老新模式。本文根据城市社区居家养老服务过程中出现的问题及需求满足，介绍了社会工作介入过程和方法途径，本文认为社会工作小组介入在社区居家养老方面的优势，利用其特殊的专业知识及技能可以有效弥补社区居家养老发展中出现的问题。 [资料来源：https://www.doc163.com]

 Abstract
China's population base is large, and it is rapidly moving towards the elderly society. The support and mental problems of the elderly have been widely concerned and highly valued by the society. The urban elderly mainly rely on their own children to support them. However, under the influence of China's fertility policy, it is difficult to meet their physical and mental needs only by relying on their own children to support them. In addition, affected by the traditional concept of providing for the aged, there are a series of problems in the family supporting for the aged. The mental status of the elderly is worrying, especially the spiritual loneliness, It affects their daily life and social activities. With the rapid development of urban construction and community in China, a new way of community-based elderly care service is also in the process of brewing. How to play the role of community-based elderly care service has become the focus of theoretical and practical research. Each elderly has different needs for elderly care service, It is necessary to construct a multi-level and multi service mode for the aged from their psychological needs.

Through extensive inquiry, collection and analysis of literature, questionnaire survey and interview, this paper investigates and analyzes the current situation, problems and needs of community care for the aged. It is found that many elderly people do not want to go to pension institutions, but choose the way of home-based care for the aged. Facing this situation, it is necessary for urban community home-based social work to intervene, Therefore, it has become the focus of this research on community home-based care. The new model of community home-based care is constructed by using social work intervention and professional division under the administrative system, and integrating talent intervention, system intervention, concept intervention and service intervention. This paper introduces the process and methods of social work intervention according to the problems and demand satisfaction in the process of urban community home-based care service. This paper thinks that the advantages of social work group intervention in community home-based care can effectively make up for the problems in the development of community home-based care by using its special professional knowledge and skills.
